Introduction: Visualisation during trans-endoscopic laser fenestration of the conchae from the ipsilateral nostril is often impaired by bleeding. Approaching the conchae from the nasopharynx may help to avoid this complication. Material and methods: The technique was first established ex vivo. A video-endoscope with a diode laser fibre inserted in the working canal was passed through the nostril, bent around the caudal margin of the nasal septum towards the contralateral conchae by a J-manoeuvre and a linear fenestration was performed. Time, laser energy, visualisation and feasibility of trans-nasal sinoscopy were recorded. Technique was used in 3 horses (under sedation and regional anaesthesia) diagnosed with sinus disease while extra recording the sinus drainage, stoma persistence and clinical results. Results: Stomata suitable for trans-nasal sinoscopy were successfully created in the conchae of all 7 cadavers. The same was achieved in 2 of the clinical cases with excellent clinical outcome. Sinoscopy was precluded in 1 clinical case by presence of an adenocarcinoma. Stomata persisted at least till discharge of the horses (7-10 days). Bleeding did not impair the visualisation as the blood flowed away from the endoscope. Discussion / conclusion: Approach used in this study optimizes endoscopic visualisation in case of bleeding. The trans-endoscopic laser conchotomy allows trans-nasal sinoscopy, trans-endoscopic lavage and drainage of the sinuses. It appears to be an effective part of treatment in selected cases of sinusitis in horses.
