    Auricular myoclonus associated with intra-abdominal botryomycosis in a dog

    A 2-year-old, male intact Dalmatian was presented for further investigation of lethargy, shifting lameness, and constant bilateral ear twitching (Video S1). Physical examination revealed pyrexia (40.8°C), cervical hyperaesthesia, and pain on thoracolumbar and cranial to mid-abdominal palpation. Otoscopic examination was unremarkable. Blood tests revealed a left shifted neutrophilia with toxic change, hypoalbuminaemia, and increased C-reactive protein (711 mg/L; Reference Interval 0-10). Electrolytes were unremarkable. Abdominal ultrasound followed by head, neck, thoracic and abdominal computed tomography revealed right ventral abdominal peritonitis, gastric lymphadenomegaly, and a small volume of peritoneal free fluid. Cytology identified neutrophilic lymphadenitis and neutrophilic exudate with bacteria. Joint fluid cytology revealed mild mixed mononuclear and neutrophilic inflammation in multiple joints. Cytology of cerebrospinal fluid (CSF) was unremarkable. Joint fluid and CSF culture (aerobic, anaerobic and fungal) did not yield any growth.Exploratory laparotomy identified a firm mass adjacent to the pylorus. The mass (7×6×5.5 cm) was resected together with necrotic omentum. No gastrointestinal perforation was noted after careful gastrointestinal tract inspection. Histopathology followed by in-situ hybridization revealed severe pyogranulomatous peritonitis with pairs/short chains of Gram-positive, Ziehl–Neelsen negative cocci surrounded by Splendore-Hoeppli material. Clusters of Grocott-positive filaments suggested a mixed bacterial component. Lymph node changes were consistent with pyogranulomatous lymphadenitis. Tissue and peritoneal fluid culture (aerobic, anaerobic and fungal) revealed profuse growth of mixed anaerobes. Aggressive antibiotic treatment (amoxicillin-clavulanic acid, marbofloxacin) was commenced. Ear twitching resolved 2 days post-surgery, and the dog showed complete recovery after 8 weeks. Eight months after finishing antibiotics, the laboratory results remain unremarkable. Some non-filamentous bacteria form characteristic colonies with Splendore-Hoeppli material in the skin or viscera, known as botryomycosis; a chronic bacterial pyogranulomatous disease that is rarely reported intra-abdominally in the dog. Although pathogenesis of ear twitching remains unclear in this case, segmental myoclonus secondary to streptococcal infections has been described in humans. It has been postulated that the antibodies directed against the bacteria cross-react with epitopes on neurons of the basal ganglia, causing a motor disorder.
