The reconstruction of the lower jaw using a vascularized free fibula was first introduced in 1989. Since then, the method has been widely used and is now considered a gold standard in the reconstruction of orofacial bone defects. We present the case of a 42‐year‐old man in whom a complex oro‐mandibular reconstruction of the mandible with a fibula free flap was performed. Patient has suffered multiple mandibular fractures as a result of motorcycle accidents. Primary osteosynthesis failed and was complicated by the development of bilateral osteomyelitis requiring further bone resection and re‐osteosynthesis with bone deficiency. Due to the reoperated field and recurrent pathological healing, a vascularized bone graft was used for the secondary reconstruction.
