A class of optimal tests for testing tumour onset times in a two sample testing problem has been developed in a recent paper by Neuhaus. These tests need estimators of the densities of the time to death distribution and are thereby somewhat complicated in nature. If such densities don't exist, which occurs often in practice since the times to death may be sacrifice times at certain fixed time points, this method cannot work.
