Purpose The effect of weakened gluteus medius muscle on the muscle system dysbalance is well described in literature. The effect of individual muscle strengthening on overall stability and muscle synergy was not studied yet. The aim of this study is to examine muscle synergy affected by selective strengthening of individual muscle segment of gluteus medius in comparison to global gluteus medius and minimus strengthening. Materials and Methods A biomechanical model of normal gait was adopted in this study. Muscle strengthening was imposed two ways. First, the maximum isometric force in single muscle was doubled to study the effect of localized strengthening. Second, all muscle actuators corresponding to principal muscle abductors gluteus medius and minimus were strengthened by doubling their respective maximum isometric force. The muscle activation and joint loading was estimated using OpenSim software Results Making stronger individual muscle unit results in its overloading while load in synergic muscles is attenuated and muscle dysbalance develops. Global strengthening distributes force between the muscles more uniformly and increases joint stability. Method of strengthening has almost no effect on hip joint loading. Discussion Biomechanical analysis provides support to complex rehabilitation therapy aimed to whole muscle system rather than targeting individual muscles.
