Flight operation can easily deviate from a normal safe process. These deviations can lead to an accident. Tracking the chain of event is foundation for safe system. Article attempts to find a way to monitor and evaluate process deviations. Bow-tie and Reason method is inspiration for safety barrier model. Barriers are classified by location and function in the system and use benefits of the barrier approach. Barrier classification and bow-tie method can be evaluated as a standard safety KPI and can be one piece of entire safety system.
