Neuroprotective Citicoline in Primary Open-angle Glaucoma

Purpose: The aim of this study was to determine whether systemic cicoline administration affects vessel density (VD) and retinal nerve fiber layer (RNFL) in compensated primary open-angle glaucoma (POAG). Material and Methods: The cohort consisted of two groups of patients with early stage POAG. Each group consisted of 10 people (20 eyes), 5 women and 5 men. The first group was treated with carteolol (carteol LP 2% eye drops) and the second with latanoprost (xalatan 0.005% eye drops). The duration of treatment was at least one year, Citicoline (NEUROTidine 50 mg/ml) was then added to both groups and taken for 4 months as an oral dose of 2x5 ml. Before the inclusion of citicoline and after the 4 months of oral administration, we evaluated vessel density in the peripapillary region (all vessels: PP-VDa, small vessels: PP-VDs) and in the whole image region (all vessels: WI-VDa, small vessels: WI-VDs), retinal nerve fiber layer (RNFL), intraocular pressure (IOP) and overall defect (OD) of the visual field. Results: There was no a priori difference between groups for these parameters (PP-VDa, PP-VDs, WI-VDa, WI-VDs, RNFL and OD) before citicoline deployment (P>0.05). IOP was statistically significantly different at the beginning of the study (carteolol 17,1 mmHg, latanoprost 14,89 mmHg). However, this difference was not of clinical significance as the values were normal. Following the 4 months of combined treatment that included citicoline, we found no statistically significant differences for either group in terms of before, and after citicoline, except for IOP, which increased by 2.7 mmHg in the eyes treated with latanoprost (P=0.05). Eyes treated with carteolol had a reduction in IOP. This change was not statistically significant. Conclusion: No significant changes in VD or RNFL were found following citicoline addition to latanoprost or carteolol.
