The autopsy diagnostics of children includes specific features, with no clear guideline available. Most discussions regard the diagnosis of SIDS. In practice, however, some cases may be falsely diagnosed as SIDS while they actually are intentionally violent acts. The aim of the paper is to describe the pitfalls of diagnosing the causes of sudden and unexpected deaths of infants using three specific case reports. The common feature of these cases was the assumption that the death was caused by SIDS, or it was diagnosed as such, and at the same time, the maltreatment or child abuse and neglect syndrome was suspected. The research method of choice was a retrospective content analysis of all medical and forensic autopsy reports of infants who died within the 1st year of life. Two of these cases were concluded as SIDS, the third one as a violent death. The forensic-medical diagnosis affects the subsequent procedures of the law enforcement authorities.
