The role of Cutibacterium acnes in the development of degenerative lumbar intervertebral disc disease and the clinical effect of microdiscectomy

Anotace anglicky

Degenerative disease of the spine is one of the most common causes of morbidity in the adult population. The first step is degeneration at the level of the intervertebral discs, often causing intense low back pain. In recent years, the presence of bacteria, especially Cutibacterium acnes (C.acnes), has been discussed as one of the causes of intervertebral disc degeneration. In our project, we will partially work with retrospective data from about 1200 patients who were enrolled in the research project entitled "Presence of the low virulence bacterium Propionibacterium acnes in the tissue of the intervertebral disc as a cause of chronic back pain". In this cohort of patients, the presence of C. acnes in the herniated intervertebral disc tissue will be correlated with degenerative changes on preoperative MRI, clinical condition, and postoperative outcome. In the prospective part of the project, patients with lumbar disc herniation who were surgically treated at the Department of Neurosurgery of the Faculty of Medicine of Masaryk University and the University Hospital at St. Anna in Brno will be included. Patients enrolled in the study will fill out questionnaires assessing their clinical status before surgery. During the operation, part of the tissue of the herniated intervertebral disc will be collected and immediately delivered to the Department of Microbiology for C.acnes detection, its phenotyping, evaluation of sensitivity to antibiotics, and detection of virulence factors, especially the ability to form biofilm. The postoperative short-term and long-term outcomes in patients with C. acnes positivity and negativity will be evaluated using questionnaires during standard postoperative follow-up controls. The project aims to clarify the role of C. acnes in the severity of clinical problems in patients with lumbar disc herniation who underwent surgical treatment. Our results may open the way for new therapeutic approaches in treating degenerative spine disease.
