    Novel strategies to investigate the origins of metabolic dysfunction-associated steatotic liver disease in lean patients

    Metabolic (dysfunction)-associated steatotic liver disease (MASLD) is a complex disorder whose pathogenesis involves (epi)genetic and metabolic factors. Although the term MASLD was coined to reflect the important role of obesity and other cardiometabolic risk factors, it also comprises lean patients or those with some alcohol intake. Lean patients with MASLD have a higher risk of comorbidities compared to the general lean population. Moreover, lean patients with metabolic (dysfunction)-associated steatohepatitis (MASH), a progressive form of MASLD, exhibit an increased risk of mortality compared to their non-lean counterparts. Thus, it is extremely important to identify the pathogenetic mechanisms involved in the development of lean MASLD/MASH. To study this, we recently established two MASH models in C57BL/6J mice that are characterized by either leaness with rapid onset of a progressive liver phenotype or obesity with delayed onset of liver inflammation. They also differ in the overall level and type (macro- vs. microvesicular steatosis) of hepatic lipid accumulation over time. Both models are based on the administration of Western-type diets (WDs) with similar macronutrient composition (21% milk fat, 40.5% sucrose, 1.25% cholesterol) combined with a sweetened drink. These WDs therefore represent an interesting paradigm for studying mechanisms of development of lean vs. obese MASLD/MASH and will be exploited in the project. The translational potential of the project is then provided by complementary analyses of the metabolipidome and bile acid spectra in serum samples from lean and overweight/obese MASLD patients from existing and emerging cohorts. Regarding the phenotype of lean MASLD, the above combined approach will allow to identify: (i) potential sex differences in disease development/progression; (ii) its prevalence in the Czech Republic; and (iii) metabolites (e.g. micronutrients, secondary bile acids) associated with disease development and/or progression.
