Development of GWAS based genetic risk scores for prediction of total and cardiovascular mortality in the Czech population.
Anotace anglicky
The aim of the study is to investigate the complex genetic predispositions associated with atherosclerotic cardiovascular disease (asCVD), total and cardiovascular mortality in the Czech population. We plan to mine existing crude data from whole genome genotyping (using Illumina GSA array) of the Czech part of the prospective cohort study HAPIEE. The cohort consists of men and women aged 45-69 years from seven towns in the Czech Republic, randomly selected from a population-based register. A confirmatory study will be conducted on the post- MONICA study. The follow-up period of cohorts is about 20 years; 25 % (12 % from asCVD of participants deceased in HAPIEE study), in post-MONICA study is total mortality 18 % and cardiovascular mortality 6 %. The results will improve risk estimates based on the known impact of traditional risk factors (smoking, obesity, diabetes, hypertension and dyslipidemia). The genetic variants identified through GWAS will be used to generate several types (unweighted - uGRS and weighted - wGRS) of genetic risk scores (GRS) to predict the risk of asCVD and total and cardiovascular mortality. The statistical analysis of the genetic predispositions will take into account not only traditional risk factors for asCVD, but also lifestyle factors and socioeconomic status. Comprehensive information on the genotype frequencies of the investigated SNPs in the Czech population can be used for future Czech case-control studies, regardless of their type and focus. The study has potential clinical implications as it could help to develop an algorithm for really early detection of individuals at increased risk of asCVD and cardiovascular mortality based on their genetic predisposition. Detection of asymptomatic at-risk individuals could allow in-time and personalized intervention and influence primary prevention strategies. The study is in line with the programme priorities.
