Mitophagy Impairment in Alzheimer's disease: From Molecular Mechanisms to Clinical Applications

Alzheimer's disease (AD) is the most common cause of cognitive impairment in older adults, however its etiopathogenesis remains only partially understood. A growing body of evidence highlights the critical role of mitophagy dysfunction in AD pathophysiology. This project, led by Assoc. Prof. Martin Vyhnálek, M.D., Ph.D., at Motol University Hospital, and co-led by Dáša Bohačiaková, Ph.D., from Masaryk University, aims to elucidate the molecular mechanisms underlying mitophagy dysfunction in AD and translate these findings into clinical applications. The initial phase of the study will focus on evaluating a panel of biomarkers associated with autophagic-lysosomal degradation in serum and CSF from patients at different stages of AD and cognitively unimpaired controls. Longitudinal part will assess changes over time and the biomarkers' potential to predict disease progression. Furthermore, the project will investigate brain-derived extracellular vesicles (BEVs) isolated from plasma to determine if they provide a more precise reflection of mitophagy impairment directly in brain. In parallel with BEVs isolation and analysis, collaboration with Assoc. Prof. Fang’s laboratory at the University of Oslo will enable examination of metabolic markers linked to mitophagy dysfunction, including NAD+ and other enzymes involved in energy metabolism. Another component of the project will involve mechanistic studies using iPSC-derived neurons from patients with sporadic and familial AD. These experiments will enable a detailed understanding of mitophagy alterations and their impacts on axonal transport and mitochondrial integrity. The research will be complemented by using cerebral organoids to validate findings from iPSC-derived neurons, including analyses of biomarker secretion. The outcomes of this integrative project will significantly deepen our understanding of pathological processes in AD, thereby facilitating the development of novel diagnostic and therapeutic strategies.
