Prostate cancer (PCa) is a hormone-dependent disease in which steroid hormones play a key role in its onset and progression. The determination of prostate-specific antigen (PSA) levels is an important tool for its early detection. However, PSA is not a specific marker for PCa, as elevated values can also be observed in benign prostate hyperplasia (BPH). Therefore, it is essential to identify additional indicators that could improve PCa detection and contribute to the selection of optimal treatment. Given the hormonal background of this tumor, it is crucial to precisely describe changes in steroidogenesis that are characteristic of PCa. Based on our preliminary results, we have found that certain intraprostatic steroids analyzed from cancerous tissue of patients after robotic radical prostatectomy correlate with tumor aggressiveness and patient prognosis. Additionally, our data indicate the presence of substances that interfere with steroid metabolism—endocrine disruptors (EDs)—which can accumulate in cancerous tissue. We hypothesize that these compounds may act as risk factors for the development of PCa. The aim of this study is to evaluate whether steroids can serve as precise indicators of disease stage before the initiation of treatment and whether they can assist in therapeutic decision-making (surgery/surveillance). Using liquid chromatography - tandem mass spectrometry, we will analyze the spectrum of steroids and EDs from prostate biopsy samples, urine, and blood (at the time of biopsy) in patients with suspected PCa. Steroid and ED levels from these matrices will be compared between PCa patients and a control group with BPH at the time of biopsy and after one year. The obtained findings could optimize diagnostic and therapeutic approaches, identify risk factors for PCa aggressiveness and recurrence, and support the integration of less invasive methods into standard PCa diagnosis and treatment protocols.
