Magnetic resonance imaging (MRI) is the most established tool used for the diagnosis, prediction, and monitoring of disease activity and drug safety surveillance in multiple sclerosis (MS). The imaging hallmark of MS consists of focal demyelinated lesions within the brain and spinal cord. Neurodegeneration, as estimated by brain volume loss, is another important component of disease progression. Although MRI plays a central role in the diagnosis of MS, misinterpretation of MRI pathology is a frequent contributor to misdiagnosis. Given that only a small proportion of new MS lesions on MRI are symptomatic, monitoring patients with MS includes regular follow-up MRIs. Compared to clinical or biochemical measures, abnormal MRI findings are the most informative prognostic markers of future disease activity. Currently, platforms that provide quantitative MRI reports for clinical practice are available. However, there are some important pitfalls associated with assessing changes in brain volume.
