The article identifies important sources of limitations of root cause analysis and finds ways how to deal with them. Responses to limitations are integrated into the root cause analysis procedure. It results in the proposal of integrated approach to theanalysis of incident causes. The approach is based on the integration of background assumptions about the structure of safety management of investigated process into a comprehensive picture and on two natural consequences of the integration: (1) classification of incident causes into four levels and (2) corresponding modification of notion root cause map. The approach is integrated in two more senses. First, it offers an integrated view of various types of incident causes. Moreover, it integrates a non-linear incident model into the root cause analysis procedure. It is shown, that the integrated approach is more universal than the original root cause analysis; that it is not less effective; and that it is not excessively complicated.
