This presentation delivered at the 11th European Stroke Organisation Conference – ESOC 2025 (Helsinki, Finland; 21–23 May 2025) reviewed the evolution of acute stroke management in Eastern Europe, focusing on system-level improvements in service organisation. It highlighted achievements in establishing stroke networks and stroke-unit capacity, implementing time-critical pathways (rapid triage, imaging workflows, intravenous thrombolysis and endovascular therapy), and monitoring quality through registries and benchmarking. The talk also outlined key next steps: reducing regional inequities, strengthening pre-hospital logistics and secondary prevention, standardising indications while shortening door-to-needle/door-to-groin times, expanding rehabilitation and long-term follow-up, and supporting implementation programmes targeting Eastern European countries (e.g., the European Stroke Organisation EAST initiative and the Stroke Action Plan for Europe goals).
