Cardiac resynchronization therapy is recognized as an effective therapeutic modality in patients with advanced congestive heart failure. Left ventricle lead implantation into the area with latest mechanical activation is important for clinical success. Transvenous coronary sinus approach is considered as a standard technique, however, in up to 2-10% of the patients this method fails. Surgical epicardial lead implantation comes as a method of choice in these cases. With close cooperation with Cardiologyclinic, we have indicated and implanted 96 epicardial electrodes from minithoracotomy approach. We have encountered reexploration for bleeding only in one case, post operative course of the rest of the group went totally uneventful. Reasons for minithoracotomy approach advantages are broadly discussed. Minithoracotomy surgical approach for left ventricle epicardial lead implantation is safe, reliable and reproducible technique with minimum compliacation rate and might have potential bene
