Noninvasive Localization of the Ectopic Focus Using Time Integral ECG Mapping

Ventricular ectopic beat focus in combination with myocardial ischemia or infarction might cause fatal ventricular fibrillation. A method for noninvasive localization of pathological region with premature ventricular activity from integral body surface potential maps (BSPMs) was proposed. The searched origin of ectopic activity was found by inverse solution to one dipole. The suggested method was applied to two patients with different ectopic activities localized by intracardiac electrophysiological studies. ECG signals from 18 different time intervals within the first 30 ms of the ectopic activity were used for computing the integral maps to find the most appropriate one for the inverse solution. For each patient 5 ectopic beats were analyzed. For thefirst patient all 90 inversely computed dipoles were correctly localized in the region of the ectopic activity, for the second patient 46 out of 90 results were not localized correctly.
