The Epileptic Microenvironment - A Novel Mechanism of Endogenous Epileptogenicity and Target for Disease-Modifying Therapy.

Despite extensive research, disease-modifying or curative therapies for epilepsy remain elusive, mainly due to an incomplete understanding of the underlying mechanisms. Traditional neurocentric approaches focusing on neuronal excitability and synaptic dysfunction have failed to yield effective treatments, necessitating a paradigm shift. Emerging evidence highlights the critical role of the epileptic microenvironment (EME) and non-cell-autonomous communication in epilepsy pathogenesis. Genetic mutations implicated in epilepsy and tumors, such as those affecting mTOR, TSC, and PI3K/PTEN/Akt pathways, suggest shared pathogenic mechanisms. Recent research demonstrated that epilepsy is driven not only by neuronal dysfunction but also by pathological non-cell-autonomous communications, where abnormal signaling from diseased cells hijacks surrounding neurons and glia, promoting epileptogenesis. The EME, a structured ecosystem comprising pathological cells such as dysmorphic neurons, balloon cells, glioma cells, and reactive astrocytes, plays a pivotal role in sustaining seizures and conferring resistance to anti-seizure drugs. These pathological cells engage in complex crosstalk with surrounding neurons and glia via paracrine signaling, cell-cell contact, and extracellular vesicle-mediated communication, leading to metabolic and ionic dysregulation, inflammation, and maladaptive neural circuit remodeling. This project aims to dissect the role of non-cell-autonomous communication and paracrine signaling in epileptogenesis and ictogenesis associated with mutations in the mTOR signaling cascade. Using advanced genetic manipulation, imaging, and electrophysiology, we will investigate how the pathological secretome of mutant neurons alters the function and structure of adjacent brain networks. By identifying key secretome components with epileptogenic potential, we seek to uncover novel therapeutic targets capable of curing the most severe forms of drug-resistant epilepsy.
