Targeting Biofilm-Forming Agents from Medical Device-Associated Bloodstream Infections: Development of Novel Compounds Against Gram-Positive Cocci for Combination Therapy
Anotace anglicky
Increasing incidence of medical devices-associated bloodstream infections, combined with pathogen resistance, limited therapeutic options, and biofilm formation that significantly reduces efficacy of conventional therapy and increases risk of reinfection, represents a serious global healthcare challenge. The project focuses on epidemiological analysis of intravascular catheter-related infections in hospital settings and, based on it, advanced preclinical development of novel low-molecular-weight (LMW) antimicrobials with high efficacy, particularly against clinically relevant Gram-positive cocci. During our research on antimicrobials, we prepared several highly active and non-toxic lead structures lacking structural similarity to existing antibiotics. The aim is to develop new compounds with potent activity against Gram-positive cocci, including resistant clinical isolates and biofilms, and low toxicity to mammalian cells. Following in vitro profiling of antibacterial, antifungal, and cytotoxic properties, the most promising compounds will be tested against resistant clinical isolates from catheters, including biofilm-forming strains. Selected derivatives will undergo mechanism of action studies; checkerboard assays with approved drugs, and evaluation of their effects on Gram-positive cocci biofilms and dual-species biofilms with yeasts, along with cellular uptake study. In vivo toxicity and activity will be assessed using alternative animal model Galleria mellonella for the most promising derivatives and their promising combinations. The primary outcome will be identification of novel LMW compounds with high efficacy against Gram-positive cocci, offering significant potential for further development and advancing treatment of catheter-related bloodstream infections. Additionally, the project will evaluate the compounds’ potential for combination therapy and biofilm eradication, which is crucial for managing bloodstream infections caused by biofilm-forming strains.
