    Identification and characterization of genetic factors contributing to inherited tubulointerstitial kidney disease III

    Autosomal dominant tubuloinsterstitial kidney diseases (ADTKD) are characterized by autosomal dominant inheritance, pathologic tubular changes and interstitial fibrosis resulting in progressive chronic kidney disease. While ADTKD is being increasing recognized as the third most common cause of inherited kidney disease after autosomal dominant polycystic kidney disease and Alport syndrome, it remains, mostly due to the technical difficulty of diagnosis, under-diagnosed and under-treated. Our past research into the causes of ADTKD has identified genetic mutations and population variants in UMOD, MUC1, REN, and SEC61A1 genes. Mechanistically, all these mutations converge in a common pathway of endoplasmic reticulum stress and unfolded protein response causing tubular atrophy and fibrosis, a process that we can target pharmacologically to slow the disease progression. Here we propose to build upon our past success in this area and work towards further understanding of the disease processes and finding the treatment. With more than 1100 families genetically diagnosed and many still without a genetic diagnosis, we aim to increase awareness of ADTKD and identify more patients in the Czech Republic and globally. Using the most advanced genetic testing and multi-omics technologies we aim to improve diagnostics and clinical care, identify known as well as novel genetic defects, increase our knowledge on the genetic architecture of ADTKD, improve our understanding of pathogenetic mechanisms of ADTKD, identify biomarkers of disease progression and find treatment. With an expanding patient registry, we will explore the natural history of the disease with specific focus on identification of factors that may affect intra- and interfamilial and gender variation in disease progression. A better understanding of the natural course of the disease and factors affecting and reporting on the rate of progression of kidney disease are essential in the development of clinical trials.
