Mapping the Path: Stratification and Progression Trajectories of Dementia with Lewy Bodies

Dementia with Lewy bodies (DLB) presents significant diagnostic challenges due to its symptom overlap with Alzheimer's and Parkinson's diseases, and substantial clinical variability. Although neuroimaging biomarkers hold promise for early detection, traditional statistical methods often overlook critical individual differences due to the case-control approach. This study proposes leveraging normative modeling (NM) and virtual connectome-based brain models (CBBMs) to improve early detection and stratification of prodromal DLB patients. NM, widely used in pediatric medicine, will be employed to compare MRI and EEG patient data against normative brain models of healthy aging, identifying early signs of cortical thinning and functional connectivity changes indicative of disease progression. CBBMs will complement empirical data by simulating brain activity based on structural connectivity, linking changes in the functional connectome to clinical symptoms. Utilizing and enriching our existing longitudinal dataset from 100 subjects on the prodromal DLB spectrum, we aim to identify and validate biomarker patterns predictive of clinical trajectories. Preliminary data on these subjects have revealed functional connectivity alterations indicative of compensatory brain network adaptation associated with cognitive benefits. By utilizing NM and CBBMs, we will study early neural changes resulting from interactions between pathology and brain compensation, and assess the clinical validity and predictive power of the detected biomarkers. The expected outcomes of this study include identifying early neuroimaging biomarkers of prodromal DLB, focusing on their link to clinical and cognitive outcomes, and aiding in individual stratification and prediction of disease trajectory. By advancing beyond traditional diagnostic methods, this study aims to address the complexities of DLB diagnosis, offering potential for tailored interventions and improved patient outcomes.
