Dermatitis herpetiformis with flame figures-an unusual case description

We report the case of a female patient with a typical clinical manifestation of dermatitis herpetiformis, and with an unusual finding of dermal flame figures in her skin biopsy. Subsequent endoscopic, histopathological, and serological examinations revealed clinically asymptomatic celiac disease in this patient. Interestingly, no peripheral blood eosinophilia or increase in serum IgE levels, allergies, parasitic infestation, and/or drug administration explaining dermal eosinophil-rich infiltrate have been identified. Local corticosteroids and the elimination of dietary gluten quickly improved skin symptoms. There are only three anecdotal reports in the medical literature documenting flame figures in the background of primary neutrophilic vesicobullous dermatoses - one case of dermatitis herpetiformis and two cases of linear IgA bullous dermatosis. Our study contributes to expanding knowledge of the morphological variability of vesicobullous dermatoses and emphasizes the need for a comprehensive clinical and pathological approach to diagnosing skin diseases
