    Background: Transcatheter aortic valve implantation (TAVI) is a widely used procedure for older and frailer patients with severe aortic stenosis. Recent studies suggest that preprocedural computed tomography (CT)-derived fat density parameters may help predict patient survival. Objectives: This study investigates the association between CT-derived fat and muscle tissue parameters at the third lumbar vertebra (CTL3) and cause-specific mortality in TAVI patients. Methods: A subanalysis of a prospective TAVI registry was conducted, including patients from 2010 to 2022 with available preprocedural CT scans. Automatic segmentation using a UNET neural network extracted CTL3 parameters, including skeletal muscle and adipose tissue densities and indices. A multivariable Cox proportional hazards model, adjusted for age, sex, and comorbidities, assessed the relationship between these parameters and specific causes of death. Results: Among 866 patients (median age 79.7 years, 49.5% men), cause-specific mortality was significantly associated with CTL3 parameters. Increased visceral adipose tissue (VAT) and subcutaneous adipose tissue (SAT) densities were linked to higher cardiovascular mortality (P = 0.024 and P = 0.037, respectively). Cancer-related mortality was associated with higher intramuscular adipose tissue (IMAT) density (P = 0.045), VAT density (P = 0.018), and SAT density (P = 0.008), while infection-related mortality was correlated with an increased IMAT index (P = 0.024). Conclusions: CT-derived fat density parameters are significant predictors of cause-specific mortality in TAVI patients, particularly for cardiovascular, cancer, and infection-related deaths. These findings highlight the role of metabolic dysfunction in post-TAVI outcomes and suggest a potential avenue for personalized risk stratification and postoperative management.
