Recently we have found a decreased frequency of the spectral peak (f) in the cross-spectra of systolic blood pressure and heart rate fluctuations (frequency range 60 ? 120 mHz) in 14 type 1 young diabetics (age 20-25 years, diabetes duration 10-15 years)in comparison with 14 healthy age-matched controls. This finding was accompanied by a non-significant (p=0.06) decrease of baroreflex sensitivity (BRS). The aim of the study was the evaluation of f and BRS measurement as a diagnostic test of autonomic nervous system impairment in young diabetics. Results: Receiver operation curves (ROC) were calculated for f, BRS, and F(z). F(z) values were calculated from equations F(z)=1/(1+exp(-z)), z=a+b1*BRS+b2*f. ROC of f and ROC of F(z)-values revealed 50% sensitivity with 100% specificity. The results indicate that measurement of f and BRS can reveal subtle impairment of the autonomic nervous system where standard tests failed.
