HMGB1, lactate and base excess: which is the best prognostic indicator in dogs with gastric dilatation and volvulus?
Popis výsledku v původním jazyce
Introduction: Gastric dilatation and volvulus syndrome (GDV) is an acute life-threatening disease occurring in large dogs. Essential for the disease development is gastric rotation and dilatation resulting in shock state. Ischemic necrosis of the gastricwall is a frequent consequence and one of the major negative prognostic factors. Methods: Blood was collected from 51 dogs with GDV prior to surgical treatment from jugular vein and metatarsal artery. Serum from venous blood was used for determination of HMGB1 by ELISA, base excess (BE) and lactate was measured from heparinized arterial blood. Results: Ten dogs died and forty-one survived. Significantly (p{0.01) higher values were observed in non-survivors in comparison to survivors: HMGB1 (median, 13.1 vs. 4.5 ng/ml), lactate (7.4 vs. 3.4 mmol/l) and BE (-7.5 vs. -2.2 mmol/l). Optimal cut-offs for poor prognosis were: HMGB1 }6.6 ng/ml with sensitivity 82% and specificity 79% (AUC 0.809, 95% confidence interval (CI): 0.671-0.907), lact
