Employee retention is a persistent challenge for small and medium-sized enterprises (SMEs), particularly in Southeast Asian economies where systemic support may be limited. This study investigates the roles of employee motivation and job satisfaction in shaping retention outcomes, while exploring the moderating effect of healthcare system support. Drawing on Herzberg’s Two-Factor Theory and Social Identity Theory, we conducted a structured survey among 385 SME employees across Southeast Asia.Results from regression and moderation analyses reveal that both motivation and satis-faction significantly influence retention, with healthcare support amplifying the satisfaction - retention link. These findings highlight how internal workplace dynamics inter-act with external institutional support. This interplay offers theoretical and practical in-sights for sustainable human capital strategies. The study advocates a combined ap-proach – strengthening internal HR practices and integrating external healthcare support (e.g., subsidized wellness programs or clinic partnerships) to improve employee re-tention in resource-constrained contexts.
