Molecular genetic causes and biochemical consequences of Congenital disorders of glycosylation II
Anotace anglicky
Glycosylation is one of the most important post-translational modifications. As a result, genetic defects in glycosylation pathways often lead to serious diseases, known as ‘Congenital Disorders of Glycosylation’ (CDGs). Over 170 genetically distinct types of CDGs have been discovered to date, and this number is likely to increase. Most of CDG types are ultra-rare disorders. CDGs cause an extraordinarily wide range of often multisystem clinical manifestations and presents with severe clinical symptoms with high child mortality. This makes the diagnosis of CDG very difficult; it is still an underdiagnosed disorder, despite the introduction of CDG screening. Patients with mutations in the same glycosylation pathway and even the same gene often have dramatically different clinical manifestations. There is still a significant group of patients with biochemical and phenotypic markers suggestive of CDG, but for whom the genetic basis of their disease has not yet been elucidated. The proposed project will improve frontline diagnostics of CDG, allow better patient stratification and select relevant cases for NGS sequencing. Exome, Genome, RNAseq sequencing of those cases will then help to establish causative genetic variants and achieve molecular diagnostic in patients with CDG. The project is important in the following aspects: 1) Finding the genetic basis of congenital disorders of glycosylation will allow early genetic counselling in affected families. 2) The introduction of new screening non-invasive methods in patients will expand the diagnostic spectrum currently available in the Czech Republic and thus enable better care in the affected families. 3) Analysis of the impact of CDG primary disorders of glycosylation on key metabolic/bioenergetic pathways will help describe the pathomechanisms of glycosylation disorders and thus provide new information for development novel therapeutic approaches.
